Connie Haxby, a Licensed Clinical Social Worker in the Commonwealth of Kentucky and State of Tennessee, has been touching lives in the field of mental health since 2005. Connie graduated from Spalding University in Louisville, Kentucky with a Bachelor's Degree in Psychology, and a Master's Degree in Social Work.
Haxby Counseling is currently not in-network with any Medicaid or Medicare plans including supplemental plans. In the event that Haxby Counseling is an in-network provider for your insurance carrier, it is your responsibility to know your mental health benefits. Acceptable forms of payment include cash and debit/credit cards including HSA and FSA plans.
