Capstone Behavioral Healthcare has been providing quality services in mental health, chemical dependency counseling, residential and community based care in Jasper county since 1968. As we start to map our future, Capstone has begun to redefine its core services and adapt to the changing funding streams that fund the agency.

Services
Capstone employs three licensed clinical psychologists and six mental health counselors/therapists. These providers work with clients and their medication management provider (if applicable) to diagnose and treat mental health concerns through a variety of psychotherapy interventions. The provider will meet with each client for an evaluation appointment and will work with the client to create an individualized treatment plan.
Capstone employs two counselors who can provide substance abuse services. Both providers complete substance abuse evaluations, for clients who are self-referred or have been referred by the courts. These providers also complete OWI evaluations. Substance abuse counselors at Capstone provide extended outpatient substance abuse treatment for clients who have been either self-referred or court ordered for treatment.
Capstone has a 6 bed group home and 16 unit apartment building that offers Supported Community Living Services. Both of these units are HUD subsidized housing units. Capstone Supported Community Living offers access to staff 24 hours a day, but these staff are not on site continuously. Capstone has a 4 bed house (JT's Place) that offers Supported Community Living Services.

We want to empower you to accomplish your goals for housing, employment and any other areas of need. Connections is a safe place to socialize and make new friends!
Capstone's Integrated Health Program provides a team of professionals working together to provide whole-person, patient-centered, coordinated medical, behavioral and social support services for Medicaid eligible adults with mental illness. The Integrated Health Program is a free service for those who qualify.

Thirteen years ago I was struggling mightily with mental problems. I had been to a psychiatrist and he put me on three drugs. I was walking like a 100yr old man and I couldn't do anything. I saw Suzanne Rozendal for the first time. She was just out of college, she didn't have her master's degree yet. I started to try to talk to her and she immediately sent me into their psychiatrist's office. She couldn't advise me on my meds because I hadn't made an appointment with her. So I made the appointment. I was taken off the old meds, and placed on new ones.

I called in to schedule a new patient appt for my child. Receptionist Tracy answered my call. Things were just okay getting information on their process to see a doctor. From the get go she seemed very annoyed to be on the phone and made me feel as though a bother to her. By the “end” of our conversation she says, “ Okay sounds good. Sounds good, I gotta go. Okay, bye” and just hung up on me. I didn’t even have a second to stop her. I was wanting to schedule and intake appt for my child and she didn’t even give me an opportunity.
