Our rehabilitation, nursing, and memory care residents enjoy comprehensive amenities, thoughtful services, and care programs customized to meet their unique needs and rise to each of their challenges.

From your first consultation forward, our experienced team will take the time to get to know you, discover your preferences and understand your concerns to create a personalized Care Plan just for you and your individual needs.
At Plainfield Health Care Center, each physician, nurse, and nursing assistant is here because of his or her dedication and impressive experience caring for a diverse range of medical diagnoses and conditions. Every short-term, rehabilitation, respite, and long-term resident receives around-the-clock licensed nursing care seven days a week.
At Plainfield Health Care Center, we've designed a specialized and separate unit to meet the distinctive needs of individuals with Alzheimer's and other dementias.
Designed to maximize resident safety, the Director of our Caring Hands Neighborhood, Dr. William Arnold, had implemented numerous different types of security protocol and capacities such as 24-hour supervision provided by our experienced and professional nurses and a keypad authorization to open every exit and entrance to Caring Hands facilities.

At Plainfield Health Care Center, we are the rehabilitation experts. Our facility was designed and recently remodeled to maximize patient independence for residents in our Stepping Forward program. In addition to some of the most advanced physical therapy equipment and our state-of-the-art rehab gym, our facility now boasts a total of 22 private rehabilitation suites.

All the staff provide well cared precautions for the facility and the residents that stay here. We go over and beyond as our goal is to provide the best care for everyone! Eric does a great job with whatever is needed with the facility!! I love working here. We are open 24/7 for visits. Easy sign in with our tablet up front, and N95 are supplied. We do our best to maintain cleanliness in the facility. As we keep growing, we do try to maintain a clean environent for our residents. I love this place!! OFFICE HRS are M-F 8:30-5, sat and Sunday 10-6

This is my second time staying at plainfield healthcare center. the first time the staff was amazing they went out of their way to make sure I had everything I needed . And they always had the best work attitudes they would always try to make you smile and keep your spirits up even though you were in pain. Now I'm in here a second time and they are still amazing . My second surgery was more painful and they are always checking on me to make sure I'm okay . I would recommend all my friends and family to come here . And the therapy people even have reunions for the people to come back and visit and

My mother has alzheimer's disease. She has been living here for over 2 years. We are so blessed. She has always been treated with respect. The staff is so caring and we appreciate all of them. This is one of the best memory care units available. I thank God that my mom has been able to call Plainfield Healthcare Center her home for the last few years. She has made several friends over time, and so have we! THANK YOU !!

This place is horrible! I only gave one star because I couldn't do 0. There were only a couple good people there! I would never send anyone there... big mistake going here. Think twice people. I' walked into my husband's room to many times to see him lay in a dirty bed, where they didnt come clean him. Left him like that all night. And when you ask about it they get rude..... I could seriously go on and on because it was the worse experience ever!!! To top it off they didnt even send the information over needed for his home health care once he was released. Think twice everyone, it's sad to see

I am guardian for several individuals in the building. The staff is always super friendly and helpful. The nurses are very good at keeping me updated with changes in condition and med changes. Social services is also great at keeping me posted. The building is clean. My clients are always looking nice and kept up so the CNA's are doing a great job as well. I would highly recommend this facility.

I finished my cna class here and it was amazing.the instructor is very positive and makes sure you know your stuff before hitting the floor. The entire staff is awesome and know each of the residents and always makes sure they're taken care of whether it be the nurses, aids, admin., etc. love it here and don't see myself working in another health care facility any time soon!
