This newly constructed 99-bed Skilled Nursing and Rehabilitation Center offers subacute nursing and rehabilitation in a beautifully designed contemporary facility. It is designed for today's seniors, with spacious bedrooms complete with private bathroom and shower. The Facility also has an abundance of common areas designed to make our Resident's stay, both comfortable and enjoyable.
I spoke with Julian who was also a Army veteran, I told him of past experiences of prior stays at a rehab closer to my house, he told me sincerely that I would be in good hands and he would see to it. From the cna's to the nurses to the dinning, all staff, directors, PT who are really good at what they do, to laundry, recreation, they all work in harmony in making sure my and others stay is as computable as possible.

As a family member, I will not recommend this place!!!! My father is a retired VA Army man who served over 30 years and I couldn't believe they made false statements about my father and told us they need to call 911 just because they saw something wrong in the xray. The ORMC ER doctor was very upset by their false statements when the hospital checked everything before they send him to that nursing home. What made them say something like that was what the doctor asked. As a family member I have called several times yesterday to speak to a caseworker and all i could do was leave the message. The

I was first introduce to Guardian because the rich community history of Guardian Care. I was even more impressed after my first visit, the staff was friendly. The residents mentioned the "at home feel" which gave me peace of mind about Guardian Care quality of service. Great experience and lovely staff.

Visited my Aunt Betty in Rm.115A. From the receptionist to the nurse's station..The staff was professional, kind and very helpful in getting me to right room since this was my 1st visit. I can see why Aunt Betty likes her new home. Once I arrived home after driving almost two hrs. I called back to the nursing station for someone to let Aunt Betty know I was safely home. Those are some SWEET AND CARING STAFF MEMBERS.

If I could rate this place with no stars I would there is some staff that genuinely care about the patients and then there are the ones who are only there for a paycheck i would never put any of my family members in there ever again they don’t properly turn the patients causing them to get bed sores to the bone that then is left untreated and then that patient body becomes septic once that happen they want to scream hospice and the patient dies i have seen this happen to two different people while in the care of guardian care both in the year 2017 its heartbreaking to sit and watch a family member
