Services
Whether you have come to a point you can no longer live alone or a loved one needs specialized care, our staff makes the transition to long-term care as easy as possible. At Azle Manner, our top priority is the comfort and well-being of our residents. By offering spacious facilities and highly skilled nursing staff we help you feel at home.
Admissions are coordinated by our Administrator, Admissions Coordinator, Director of Nursing and Social Services Director. All referrals and inquires are reviewed by our team. Once the necessary medical assessments and financial information is received, a prompt response on appropriateness for placement can be given.
The quality care we provide is accomplished by staff that is trained to care for the individual needs of each resident. We provide care according to each resident's particular functioning level, ranging from early dementia to medically complex needs. A Licensed Registered Dietitian conducts ongoing assessments of each resident's nutritional status.
If you've just had hip/knee replacement surgery and returned home safely, it's now time to think about physical therapy. At Azle Manor, our committed specialists are devoted to top-quality care and unmatched results in a variety of different rehabilitation areas. Our therapists work towards setting goals by completing well-thought-out activities and objectives to achieve results.
Medicaid is a cooperative federal-state program designed to provide assistance to low-income people. It has become the major funding source for long term care. Medicaid is administered by the states under broad federal guidelines. Reimbursement rates per day of care are also set by the states.

My Dad was at Azle Manor (he passed away). My Step- Mom is still there. Words can not express how awesome this nursing home is. I can give many examples, but the latest is about the physical therapy department. Don manages that department and he is great. Jessica recently worked with my Step-Mom and when she was done, she painted her finger nails. Not in her job duty description, but this is what caring people do. When my Dad was alive, they also took such good care of him.

Excellent place for elderly residents. My Father in Law is there. They take such good care of their residents. Big shout out to Kip Kruger, Administrator, Bridgette Maclin, Laci Gray and the Nursing staff (Amanda and others) for all of your hard work. We, as relatives of your residents are appreciative of all that you do. We love it that you provide social activities, and concerts for the residents, as well.

Nurses did not cap a central line and my father got infected from it. He was physically hurt by an aide during a shower. DON retaliates against patients family's who complain and will hurry discharge to home to get rid of anyone complaining. My father was moved to a lesser room without a bathroom for him to shower after talking with her and faulting her staff for sub standard care. He is now at Garden Terrace receiving great care.

This facility is wonderful. We had to put my Dad and Step-mother in Assisted Living. We originally put them in a home in Weatherford, Tx and they were so unhappy. We looked into Azle Manor and was very impressed, so we had them moved. They have been there for 3 months now. We are so happy we made the change. They are an excellent residence for our folks. Shout out to the Administrator, Kip, Bridgette (who is awesome with regard to helping with Medicaid and financial stuff), The Nurses (especially Mandy), and staff are very caring and helpful. Also, the grounds are beautiful and deer often

You guys have finally put someone on the front door who knows what they are doing! Ms. Judith is awesome! She greets me with a hello and a smile everytime I come in! The other girl never looked happy, was very rude and had no manners. I am very pleased with Judith’s professionalism and compassion. She makes sure you understand things and helps the residents out as well. Good job azle manor! I am very happy my mother is at this facility now!

I have been a nurse at Azle Manor over a year and a half now and I enjoy it so much! I love all of my residents like my own family and I can promise you that myself as well as the rest of our staff go above and beyond daily to make sure our residents get the best care. Our new management is absolutely amazing and making some great changes that are benefiting not only our staff, but our residents. I am so thankful to get to work here and take care of your families! ❤️

Having to place our precious 98 yoa aunt in a nursing home was something I dreaded, but when the time came that we were forced to do it, Azle Manor really came through. From the aides to nurses to therapists to administrators I can give no higher thanks and recommendations. Not only were they kind, loving and patient with our aunt, but also with us as we adjusted. Our aunt passed away and they even made that experience beautiful.
