By far one of the BEST skilled nursing facilities in the area. All departments from rehab staff, nursing, housekeeping, to dietary the entire facility is filled with caring and passionate employees. I've had multiple friends/neighbors/family members here and would highly recommend!.

It's like someone else said in this review if I could give it 0 I would. I wouldn't put my dog into this place. My brother had a bracelet saying Fall risk he had a fractured vertibre and had a brace on his neck.he was always up and about when I went into his room. I even video taped it because it happened so often I understand that they are low staffed . but he was in the hallway one morning walking down holding the railing because he didn't know where he was going or where he was A woman visiting her mother found him that way and took him to the nurse's station. My mother has been in and out of different nursing facilities in Suffolk County for the past 3 years. I have to say Momentum has been the best one. My mom received wonderful care, excellent rehab team,, they listen to your concerns and they care. I have not had many good experiences with several other facilities. Especially during the pandemic, Momentum does their best to accommodate families and my mom was very happy there. I would recommend this facility to my family and friends.

Staff claims they are short handed, so short handed that my best friend has been sleeping in a chair for 2 weeks because they don't have time to put her into bed, I just got off the phone with her,she needs to go to the bathroom, no one has come though she has called at least an hour,she needs a drink, they refuse to give her a drink because they don't want her to have to urinate, 3 weeks ago she fell out of bed,had to go to the hospital, add insult to injury I spoke to my Dad this morning, my Aunt was there also,come to find out she fell out of bed 3x's. I want to send a heart felt recommendation for this facility. My mom was mentally not ready for a nursing home. Physically was a different story. The staff, was so supportive and so caring that she was able to adapt very nicely. The facility has so many programs to offer stimulation and community. Mom was a recreational therapist in her prior life and the recreation team loves her input. She feels important. How can I say it better? She feels like she makes a difference in the lives of the staff and community around her by celebrating their milestones, and they hers in turn! Each of the departments

This is by far the best facility that I have ever been associated with. I have been witness to a great deal of residents who go from medically unstable and non-functional to healthy and independent! The staff here is nothing short of miracle workers. They have been especially incredible over the last year plus dealing with the Pandemic and still caring for their residents at the highest levels. If you or someone you know needs a great rehabilitation center, look no further than Momentum at South Bay!!

My 86 year old Mom was admitted to Momentum Rehab after having a 10 day hospitalization for anemia/blood loss. She needed to get her leg strength back since she spent her hospital days mostly in bed and sometimes in a chair. Through Medicare you get 20 days of paid rehab care. I never thought she was going to be there that long so I wasn’t really concerned because with her blood count up she had more energy and strength to progress to walking with a walker as she did before she was hospitalized.
If you are looking for your loved one to come home in a timely fashion this is not the place! Unfortunately

My mom entered rehab at momentum after a week and a half hospital stay. She could not return to assisted living without rehab services. Momentum has yet to do the necessary paperwork for her departure. They are dragging their feet…assistTed living has already okayed her return ….momentum is a for profit facility trying to rack up the costs ! Hitting walls wherever we turn to ,

So very sorry to have to say, but the rehab was extremely disappointing. Very disappointed with the people In charge. I would never state names, but I pray they never had to experience what my family went through in there facility. I have no idea about the patients that are there long term, but my advice would be to never put your family in this rehab. This was my family’s experience, we can only speak of our experience. A word I could use was neglectful medically. Not equipped to handle anything medical. All I can say is shame on them. Maybe from this they can be honest with themselves and try

Having been a patient here at Momentum Rehab for almost 3 months now. I think I have been able to get a keen perspective into the facility. I shattered my leg and had to undergo a few surgeries, recuperate, then go for another surgery. The Wound Care team has tended to my wounds, even my grafts, with the greatest concern. He has worked patiently with me as I healed many times in excruciating pain. The Physical Therapy and Occupational Therapy I have received here has been beyond what I expected.
