At Smithtown Center our trained staff are there to care for the residents 24 hours a day, 7 days a week, ensuring that Smithtown Center feels like their new home away from home.
At Smithtown Center, in order for our residents to receive the highest level of care, we provide a variety of services onsite. These health care professionals include Dentists, Ophthalmologists, Podiatrists, and certified wound care professionals. In addition we also provide onsite laboratory services, x-rays, ultrasounds and state-of- the-art diagnostic equipment.

My father was cared for recently for the second time at Smithtown Care Center. The nursing and physical therapy staff are all wonderful and caring. During his stay, his diet improved, his blood work stabilized and he thrived. He had no skin issues or breakdowns. There are activities but he did not participate in them but he appreciated the attentiveness of the staff, especially the aides who cared for him. I am not one for writing reviews but my experience with Smithtown center has compelled me to share my experience. My mom has been a long-term resident for many years, and she transitioned to the facility after having a stroke and three brain operations. My mom went from a fully independent women who was active in the community to being completely dependent for every aspect of her life. I had gone to many facilities to find a place for mom and to ensure a quality of life that she deserves. I was appalled by what I saw and walked away feeling very sad. My mother has had her third visit to this facility. Though there were some small bumps in the road it was a great experience. Even those small bumps were addressed immediately and resolved without issue. I would recommend you let your loved ones stay here in their time of need. My Mom has been to some horrendous facilities, this one was a blessing.

I cannot say enough good things about this facility. My 92 year old mother suffered a major stroke and was admitted to the rehab floor several months ago. Everyone my Mom and family encountered at Smithtown Rehab was extremely competent, compassionate, and dedicated to my mother's care and recovery. Mom was there for 6 weeks and her care had slowly dwindled down to zero. She would come outside for a visit without any pants on, her leg brace was either put back on incorrectly or not even on. They evidently don't watch the patients closely enough to know what they are there for. The PT's should have known better on how to remove an immobilizer the correct way, that is what you would think they are trained to do. What a mess. This was not my first choice and it shouldn't be yours either.

After having experience with several other Rehab/Nursing across Suffolk County, I highly recommend this facility to anyone. During a time of uncertainty, my mom was treated with absolute utmost respect, dignity and professional care by the entire facility. My husband just completed a 61/2 week stay after being in and out of hospitals and another rehab facility for 5 months. It is nothing short of amazing how wonderful he is today! The staff was truly invested in his recovery. He had very complicated medical and physical issues that were appropriately addressed. His physical therapists (David and Cilffton) were amazing!!! He was not always easy but they pushed him and he is better than ever. The recreation staff (Vanessa, Yennifer and Alicia) greeted me every day (yes I visited every day) with a smile and facilitated my visitation even in the face

My mother is a patient in the sub acute rehab center. The facility and staff have been spectacular. It's a difficult situation for our family but the Smithtown Center has been great. My husband was at Smithtown Center for three months. When he arrived he was basically immobile couldn’t walk couldn’t speak couldn’t eat. The physical therapy group was fantastic. the nursing care, the food service , the nurses aids , everyone was amazing. The care they gave my husband was extraordinary. The physical therapy checked on him regularly just to make sure he was doing OK. The nurses and aids made sure that he socialized, came out of his room just took excellent care of him.
