Affinity Skilled Living and Rehabilitation Center, is one of Long Island's newest and most progressive 280 bed residential health and rehabilitation facility. Affinity is situated in the lovely neighborhood of Oakdale, Long Island. Short term rehabilitation and sub-acute care are comprehensive inpatient services designed for someone who is recovering from an acute illness, injury or exacerbation of a disease process.

In addition to offering state of the art rehabilitative, medical and complex medical care in comfortable, spacious surroundings, Affinity's professional and compassionate staff members provide round the clock skilled nursing services to all of our customers.
We accept a variety of commercial insurance including Blue Cross & Blue Shield, Atena, Agewell, Affinity, Cigna, Elderplan, Healthfirst, Humana, Quality Health Plan, United Healthcare and Wellcare. The insurances are updated frequently so please call the Admissions Office if you have any questions or do not see your insurance listed.
Protestant Services service presided over by Rev. Brian Noack Pastor of Saint John's Lutheran Church. Jewish Holiday Services celebrated and led by Rabbi Dr. Steven Moss from B'nai Reformed Temple. A wide range of exciting culinary experiences are available.

The Recreation Therapy department provides a modern array of programs including guest chefs from the local area as well as cooking series that use new techniques featured in such publications as "Food & Wine".The Executive Chef at Affinity also comes out of the kitchen and conducts cooking demonstrations.

I'm not an expert, but I do not think it is wise to give old people that barely eat or drink a laxative every night. They cant walk, end up making a mess , PLUS what is it doing to them health wise. Electolytes, etc. My father was on BP meds and lasix. Dehydration? Not good. When I said I didnt want him to have it they would not comply saying they didn't want them" bound up".

Compared to other skilled nursing facilities, this is a great place to work. I truly enjoy coming to work as a nurse and being there for my residents, especially when families are unable to visit due to covid/quarantine. If you’re looking to bring your loved one to a skilled nursing facility, look no further! You have the executive director leaving reviews about the place he works at to build up reviews that is beyond funny. Hey Eli what happened to the guy that got drunk and you refused to make a report about or how about Dr O who had his license suspended and can’t not write narcotic prescriptions and had had 4 patients due in his care this month. I wait how about the patient you threw out of your place gave him 500 hundred dollars and he sided in the cold because he had nowhere to go. What a great place. Please if you love your family members do not send them here

Affinity has uplifting physical set up for the geriatric population with lots of sunlight and talented staffs.
I have been working on all different skilled nursing facilities for many years and when I see Affinity staffs care for residents with full heart and good skills make me happy to work there. It is difficult time for all of us in this prolonging pandemic situation but we share our talents and love with our residents and families that’s what Affinity is now.

Affinity is a great place to work. I truly am grateful to have the opportunity to work in this facility. When questioned, I respond with pride when I say “Affinity Skilled Living and Rehabilitation Center”. It is super clean, resident rooms are very spacious and the atmosphere is very home like. With no hesitation, I would place a family member in Affinity.

I would not recommend this place at all! They left my mom in the same position all day long. The aides are nasty, one got rough with my mom and yelled at her roomate. I pulled her out after about a week. They lost all my mothers clothes, I inquired about finding them many times, the social worker and the head of nursing said they would look into it and call me back , I'm still waiting for that call 4 weeks later. Even when you call there the people who answer the phone act like your bothering them.
Do NOT bring your loved one here !

My mom has been here for 3 years and she loves it. A month after she was admitted for rehab she decided she wanted to stay for good. She loves her nurses , she likes the food and it’s been great to finally get to visit her again. The therapeutic recreation department is doing an exceptional job.

I wish I could leave 0 stars but unfortunately I have to give them 1 star. My mom had made arrangements to drop off Christmas gifts for my grandma on Christmas Eve, have someone bring the gifts up to her room, sit with her while she opened them and take what needed to be labeled. They brought up her gifts, but nobody had her open them and therefore nothing has been labeled. I've been trying to get in touch with the nurses station on her floor for over a half hour and just nobody is answering the phone. They don't go to dinner for another hour so what has everyone been doing for 30 minutes? I genuinely
