With an experienced interdisciplinary team of physicians, nurses, social workers, rehab specialists, dietitians, and therapeutic recreation specialists, our staff will offer customized care planning for each resident to help them achieve functional independence. Social workers assist residents and families through the admission process in addition to coordinating preparations for discharge.

Based on the unprecedented challenges of the COVID pandemic, this nursing home deserves a shout out. I was fortunate that my parents were accepted there two years ago (my mum passed away on their second anniversary recently) into a spacious and large double room a few months before the pandemic ravaged nursing homes and I got to see the place in full swing! Islands, due to its extremely severe pandemic restrictions, continued to soldier on during the pandemic using Facebook video calls for residents and their loved ones to communicate between infrequent outdoor visits. I have been here 6 months the care is very good Pho very prompt when you ring the bell unlike some places I especially would like to commendcna apearlon the first floor she does an exceptional job and deserves a raise she goes above and beyond but overall the place is clean and well taken care of the theeapy was great better than Southampton rehab! The food is overall nutritional i recommend the cod fish sandwich they even made shrimp scampi one day which was delicious i ORDERED seconds not bad for rehab

This place is absolutely horrific. The room my grandfather was put in was filthy, filled with dust bunnies and food containers. The alarms on the beds of the patients go off at the slightest movement, although they're worthless because while sitting with him it went off 4 times and not a single person came any of the times. Not ONE. None of the staff here deserve their medical licenses and I advise anybody to stay away.

My mother was recently placed there for rehab after a bout with pneumonia. She arrived on Friday night and I had her out on Wednesday after 3 full days. The first floor is NOT adequately staffed for the acuity of patients. The staff was trying hard but overwhelmed. No one at the desk. I saw the son of a client almost in tears cause his mother was not being adequately cared for and he was trying to move her out. I saw 2 families waiting to speak with nursing supervisor on duty along with myself. Thank goodness my mother is alert and oriented and able to ambulate. On Saturday night she was served

Please dont give a copy of your license or certifications when walking in to filling out an application. These important documentations should be collected during the hiring process. The people that have No criminal history, Never been arrested and enjoy helping people while having experience wont get an interview if they worked at too many facilities in the past. When you walk in to fill out an application the nice receptionist will ask you for a copy of your nursing license or certification immediately. If they see that you have worked for a lot of healthcare facilities in the past they wont

May god strike me down if I'm lying. My mom waits 40 minutes plus for a call bell to take her to the bathroom because she MUST be assisted. She is there after a revision of a knee replacement .They do not properly record the patients charts, day nurses play favorites and retaliate, you complain, you wait, multiple patients tell me the same thing. Its like a conga line in the morning, nurses dont come to you with your meds, you are wheeled, if your lucky , on a line in front of the nurse with a medication cart. After reading all the positive reviews about this facility I was very interested to have my loved one moved from a negative hospital experience here.
The people were very nice from the first telephone conversation. So I tirelessly worked on getting him moved. Eventually the first negative experience occurred when an employee that works only the business hour shift spoke with me on the phone and I made a mistake of mentioning he may have been accepted at St. Charles - my initial pick before researching the facilities.
The list provided at the hospital was several pages long and we were instructed

My dad was there in May. Very disappointed with the care my dad received there. He was there less then 24 hrs and left along on the toilet and fell. Rushed to the hospital. Then had the nerve to ask for $500 to hold his bed. He had come from Brookhaven hospital which has the worse drs. And was in bed there for 9 days. Nurses were not around much and dr there didn't see my dad was dying right in front of them. Ask for dr to call me, I'm still waiting. Food disgusting it was just an awful place and feel so bad my dads last days were spent in this hell hole

If I could give zero stars I would. One star will do because the place has nice community rooms. Having said that, my grandmother was sent here after an operation at Stony Brook. She was there only about 10 days. She was otherwise healthy and still took care of herself before her time here. From day one we started noticing the lack of attention from the aides and nurses. She actually told me she was afraid to ask one person specifically for help because she was very mean. I also witnessed this person telling her roommate "what's wrong with you? Call me when you are ALL done". She needed help in

I recently visited my aunt at this facility while she was recovering from an illness.
The facility staff overall was pleasant and accommodating to their patients as well as the visitors. Plenty of parking around the facility as well.
The patient rooms were very spacious and could accommodate large families like mine. They also had quite a few family rooms were you could visit with the patient.

My father spent a couple of weeks at Island Nursing & Rehab to recover from foot surgery. For the first few days, they were great, giving him all the attention he needed. Then they dropped the ball. Twice I arrived to visit him and found the bandage on his foot had unraveled and his bare incision was resting on the floor.
