Services
Saint Francis Rehabilitation and Nursing Center is a 137 bed Skilled Nursing Facility located in the Grafton Hill area of Worcester. Our campus consists of 63 long-term care beds, including a 16 bed men's only unit. The center has only private, semi-private and grand semi-private rooms.
We have five nursing units, 53 private rooms, two (2) small neighborhood Alzheimer's Units totaling 37 secure beds, 35 Sub-Acute and a separate Adult Day Health program that can provide services for up to 60 participants per day.The skilled nursing facility has a beautiful Chapel and celebrates Mass six days a week.

Full Time, Part Time - As a Registered Nurse on our Center you will be an integral member of the team ensuring the health and well-being of residents in the Transitional Care Unit (TCU). Our state of the art program provides medical and therapeutic. Full Time, Part Time - The licensed nurse provides direct care to the residents and is responsible for the accurate and timely distribution of medications.

This unit offers a home-like secured environment that is specifically designed to lessen the stresses of everyday life and provides the therapeutic environment necessary to enable a person with memory loss or dementia to remain as independent and engaged as possible.

My mom is currently at St Francis recovering from a broken hip, one that she received while at a hospital emergency room, if you can even imagine. My first impression of St Francis is not what I would call good, the room was hotter than an oven and I felt like it would be a very dreary environment. But to be honest I had just spent 60 hours in the CDU room in a chair waiting for my mom to recover enough to go back to a rehab facility and not the terrible one that she was in previously, so I was quite tired and moody. I was approached by CNA Lisa fairly quickly. She was pleasant, I was not. Looking

If you care at all about your family members you will not send them to this place. My father in law was here and has missing clothes and shoes, wasn’t showered or had his face shaven. When sent home they didn’t adequately transport him into his vehicle. They sent us home with another patient’s medical information (HIPAA violation). They set him up for a medical procedure at the hospital without telling us which we were fortunately able to cancel. Awful place that needs to be investigated by the state.

Terrible experience getting in touch with a resident. They schedule appointments and don’t keep them and it seems they only do the minimum to keep being able to bill the insurance companies. Hard enough to contact family

My grandmother is currently here and hasn’t been there long but the experience is terrible. It’s very gross inside. She’s already caught the nurses trying to get into her computer she has there. They only shower her once a week, every Wednesday but for the past two Wednesday’s they’ve told her she can’t get a shower because they have no hot water. TWO WEEKS without a shower. Most of the nurses have a nasty attitude. I will forever wish i could bring her somewhere else or to live with me because this place is horrid.

Absolutely disgusting facility for your loved one. Had to report ROACHES in my fathers room on different occasions, the facility smells appalling, poorly maintained, and care for your loved one is overall horrid. The administrator Katie is despicable. GO ELSE WHERE PLEASE FOR THE SAKE OF YOUR FAMILY!
