We believe that transparency and strong communication is the key to successful patient encounters. Below you will find the Oregon Pain Center Opioid Agreement, we ask each patient to sign the agreement prior to prescribing and managing their medications. Please let us know at the time of your appointment if you have any questions or concerns.
The caudal canal that passes through the tailbone or sacrum is the extension of the epidural space from the base of the spine. Usually there is a small opening at the end of the tailbone called the sacral hiatus. Placing a needle into this opening and injecting through it is called a caudal epidural injection.

Absolutely love Dr Albert Rodriguez and his staff at Oregon Pain. I have had severe back and hip pain for years, and finally it is under control and my quality of life so much better. Excellent procedures , the right medication. Albert takes time to discuss and listen to your concerns. Friendly staff , like a family, .No need to suffer with pain.. I am 44 years old, I am an anesthesia provider, and a patient with degenerative disc disease. I have had 2 spine surgeries and one ablation prior to meeting the team at Oregon Pain Center. Caitlin, the RN scheduled all my appointments like a pro. Albert, the Physician Assistant, listened to every word I said, and treated me respectfully, as a patient with legitimate pain. Dr. Mambalam did my injections and RFA beautifully and I got 90% relief (so far!) Even working in healthcare, I felt sometimes as though I was not believed. I felt belittled, and as though I should just stop asking for help. OPC

I have been suffering from auto immune problems for years. One of them had caused so much pain to my neck. My past doctors would prescribe pain meds and muscle relaxers. Which worked temporarily. When I started coming to The Oregon Pain Clinic it changed my entire way of living. The procedures they offered had never been offered before. I was nervous but the staff were so helpful to put my nerves at ease. After a 2 procedures to my neck I am now able to move my head again. I’m so grateful to the is place. Thank you.

Terrible experience. They do not sedation for RFA. They might give you zanax. You meet the physician 5 minutes prior to your procedure.
They are much more interested in your money than actually helping you.
They will make you reschedule if you don't have your copay (i had cash, they wouldn't accept it, debit card declined) and will charge $100.00 late cancelation fee when you can't make your appointment but can't get through to the office
(first appointment of the day, answering service available "only for emergencies")
Did they call to see how I was doing after RFA? Nope. Awful office.

This place is a joke. Stay as far away as possible. They are in it for one thing and that's money. Half the time when I went in. I saw a inturn and never the Dr. Was never there more then 5 minutes. I had two steroid injections in my spine from him. Neither was effective. They have unreasonable rules about having someone accompany you, sit in the waiting room, then drive you home. This is a steroid shot, not an opioid. Most clinics just require someone to show up at the end and drive you home. The whole office is very customer unfriendly.

After many months of severe back pain from arthritis, I found Oregon Pain Center and Dr. Praveen Mambalam. They quickly made an appointment, checked my insurance for me and I was on my way. I met with Albert and Dr. Mambalam on the first appointment. After an extensive exam, we talked and discussed a plan. I left with an appointment for my first test injection and a feeling of confidence that I had found the right team.
My insurance requested two test injections and then the appointment for ablation. I am five weeks post ablation and no longer have the intense pain. There is an ache but

Dr Jorgensen and his staff are true professionals with great attitudes and willingness to help their patients and have always treated me with respect. Dr. Jorgensen has been very informative and has always presented me with all the opportunities and let me choose my path. This clinic is my third clinic in 15 years and it is significantly better than the others I've been to, both the doctor and the staff. I'm not having injections at this point, but if I needed them in the future, I would have high confidence in Dr. Jorgensen's ability to treat me.

OPC took me in when other clinics claimed I was an addict, and worked with me to get pain free in 7 weeks. Their staff is more than courteous; they’re like a family. I love Dr. Mambalam and Albert’s rapport, in working with me they did what other clinics couldn’t.. give me hope for a pain-free life without lots of medications. The injections I had were the least painful I’ve ever gotten and I am glad to be on a minimal medication regimen. Thank you so much Oregon Pain Center!

I had a great experience at the Oregon Pain Center. Dr. Jorgensen was professional, knowledgeable, and took time to answer all my questions with a high degree of care and wisdom.

I went in for an epidural spinal injection for a herniated disc between my L5 and S1. My anxiety was a 10 out of 10 going into the procedure. The actual procedure was so not a big deal, I would say the pain associated with the injection was a 2 out 10. Again, it was way less painful and stressful than I thought. My positive experience is a direct result of the experience and professionalism of the both Dr, Jorgensen

It's a small private practice. The staff is great, always on time. I felt the doctor was knowledge and fast.
I've used him several times, prior to neck surgery and trying to extend a herniated disc surgery through injections of steroids.
I gave him three chances and won't be going back, the doctor knew of the pain, he knows I'm able to take certain pain meds per my company policy but even though I was out of work for a month he is afraid of getting sued when it comes to pain meds.
I knew this after the first visit so i made other arrangements. I asked to see him for

Don't go to this clinic unless you want a pain doctor who you will only talk to at your first appt, is condescending and abrasive, uses unethical business practices and basically seems egotistical and greedy.
If you do go see him, expect to only talk to his PA or other staff. Last time I got a shot in my lower back, they had me turned facing the wall and the doctor walked in and did the shot and left within 2-3 minutes. Didn't even give me the chance to turn around to ask anything. He clearly doesn't like his profession.

I went here 2x both times i had Negative experiences, Dr Jorgensen was very rude to me ,i felt like he really doesnt want to be a dr. Ive had 4 back fusions, i have arthritis and other pain condtiions and all he could tell me was get off the meds and thats the only thing that gives me any Quality of Life. So he did nothing but got paid both times from my ins which is not right! Very wrong!

I've seen both PA David and Dr. Jorgensen. They are amazing. They utilize a multidisciplinary approach to help with functioning and quality of life. They take the time to listen and want you to be involved with your treatment. The staff is impeccable and know you by name.

This is a legitimate pain management clinic. If you are trying to get narcotics this is not the place for you. If you want to have a better quality of life and use the treatment plan they give you with minimal medication use then you will do great here!

Dr. Jorgensen and all the ladies in the office are great. He is the first doctor it seems, that's actually listened to MY opinion about my body. He's always kind and can even tell if I'm in a good or bad mood when I walk in. They all know me by name, and I even got a round of high fives last time I left because I'd lost some weight. I'd recommend the oregon pain clinic to anyone who asked.

Don't pass judgement on people of color! A Dr. job is to help clients with pain relief as much as possible. Patients personal life is not for the Dr. to pass any kind of judgement!
Some Dr. need to get themselves together & do their job! Don't look down your nose at your patients! We're real people with a lot of real pain going on & the last thing any patient needs is a condescending attitude! I would Never recommend this place to anyone! Ever! Dont pay the 200 fee they charge u then tell u they cant help u or accept u at their clinic then say its an administrative fee thats non refundable shady business calling better business on them

Dr. Jorgensen is awesome! He really takes the time to talk with you and get your full history. He has a wonderful bedside manner and is very friendly and professional.
