Founded in 1949, the New York State Health Facilities Association (NYSHFA) is a statewide membership organization of more than 250 licensed nursing facilities. The membership includes individual and multi-facility operations of proprietary, voluntary and government sponsorship. NYSHFA members, who care for nearly 45,000 New Yorkers, are dedicated to the highest standards of care and to the quality of life of their residents.
How do I place someone in your facility? A. Please call our facility to schedule a tour and complete an application for admission. We will need to obtain medical information from the prospective resident's physician or hospital discharge planner. Does the facility accept Medicare and/or Medicaid are they different?
The staff of Capstone is aware that an admission to a health care center is often unexpected, hurried and emotional. Our professionals are ready to provide you with the information necessary to help make an informed decision about the most appropriate care and services you or your loved one require.
Capstone is a certified residential skilled nursing facility, providing short-term rehabilitation and long-term care to individuals with health conditions that require higher levels of medical and nursing services. Under the supervision of Capstone's medical director, a team of physicians cares for each resident.
Capstone has social workers on staff to help the resident and their family members adjust to placement. Working with the psychosocial needs that may arise during the course of the individual's stay at the facility, the social worker is not only a source of emotional support, he or she also assists with any necessary adjustments to facility living, mood, financial, and individualized concerns or family requests.

I'm the 3rd Floor RN Unit Manager, Ive been a RN for 15 years and I've worked in LTC for 25 years. No lie, hands down this is the best facility I've ever worked at. My co workers are phenomenal and since the day I started back in September I've felt valued and felt like family. Management has gone out of their way to ensure I have everything I need to do my job and to help me do my job the very best that I can given the current COVID conditions. My friend was in this facility for a short time, what I observed was very disturbing.. The facilty claims understaffing is the reason for the overwhelming neglect of the patients. If understaffing is the cause and reason for this, then this facility should not be operating at full capacity.. All I can say is if you have any love and compassion for the elderly. Please..DO NOT PUT YOU LOVED ONE IN THIS FACILTY!!

My Mother has been there since September 14th and my sisters and I have nothing but good things to say!! My Mother has been well taken care of and her rehab has been fantastic!!! She walks(with her walker) better than she ever has!! The Staff has been very caring and helpful and she really likes them. I would highly recommend Capstone Center Rehabilitation.

My mom was there at the end of May 2021 do keep in mind it was for less then 48 hours and during COVID-19 time. But this is the big reason why they get this high as well. We could not see my mom yet and she was in bad shape but the time my dad talked to her we knew something was wrong so I called the nurse they kept a good eye on her that night next thing I know she is of to St. Mary's and has what she had when she left the last hospital she came from. So I was so happy that they stayed on top of things.

Bad they need to be retrained on what a health care proxy is.. there allowing a sibling to determine who can visit we been giving her clothes and food and flowers they take them then tell us she will get it after 3days weeks go by we call and they tell us her health care proxy said don’t give her anything. Worse place ever.. so call has dementia but she is well of swear of the situation.. JESSY HER DAUGHTER NEED HELP WHO WOULD WANT THEY MOTHER TO SUFFER.. THEY NEED MORE PEOPLE THATS SPEAK DIFFERENT LANGUAGE

We needed to get my Grandmother into a Nursing Home ASAP. Capstone was great at taking her in (as it was the only available bed). That's where the positive energy stopped. She was barely taken care of for the 4 months she was there. Each week her clothes and personal property went missing. We even took her clothing home to wash in hopes it would help but they still vanished. Teeth were gone as well..and never replaced. She lost alot of weight because they are in such a hurry to serve food that she wasn't assisted in eating. Also, she wasn't groomed well. Her hair was always greasy and if the person
