Collar City Nursing and Rehabilitation Center is a 120 bed skilled nursing facility, conveniently located in the beautiful upstate New York Capital Region. Services
The goal of the Collar City Rehabilitation Department is to help our residents to meet the goals they seek to achieve, whether it be to return to their home in the community, to improve their strength and endurance, to complete daily tasks more independently, to decrease pain.
Our team of professionals include Physical Therapists, Occupational Therapists, and Speech-Language Pathologists to help our residents meet their goals for rehabilitation services.Our rehab suite offers a kitchen set-up with stove, refrigerator, microwave, and washer/dryer. We have a full bathroom with tub set-up.

We encourage prospective residents and their families to schedule a tour and meet our knowledgeable staff. This will allow for a smooth transition from the hospital setting. Our Admissions Coordinator will diligently guide you through the admission and discharge process. Our clinically certified Community Liaisons work closely with patients, families, hospital staff and physicians to determine the most appropriate treatment and setting.
Collar City Nursing and Rehab takes great pride in our dining program. Our Food Service Director is unmatched in offering a program that provides delicious, home-made meals, in a restaurant style atmosphere. Our experienced chefs and courteous dietary staff ensure that each meal is a pleasurable experience.

This place is horrible, it needs to be shut down. They are not equipped to take care of patients. They did not have pillows for me to elevate my leg after having surgery. The rehabilitation dept is not properly supplied. They did not have crutches for me to use. The nurse's don't show compassion. When I pressed the call button for the nurse it always took 10-15 minutes for them to ans the bell The executive director basically just gives alot of lip service. I had bugs in my room. This is like being in hell & I was thrilled when I got out to go home. Stay away from this place.

I went to visit a friend that had recently broken his pelvis. The staff was so kind & attentive to him. Everyone was very pleasant from the receptionist, nurses aids and nurses. The patients seem happy & well cared for. One patient even sang me some Broadway hits. Even though my friend is still in pain he said that his therapy session went well.

I had a great visit with a family member since the building is open. The check in process was quick and I got a fast covid test. The visit area was clean and had great social distancing. We had a private area. The building is undergoing a lot of nice renovations. The staff were happy and friendly. They even had homemade cookies for the visitors. Nice touch. Can’t wait to go back. There is a online booking system for visits and they have iPads for residents to use for FaceTime.

The Diamond is a great place to go for rehab. The new owners are wonderful, they actually listen to the residents and staff. They are in the middle of a beautiful renovation. They are investing in our community. The staff are great, a lot of the nurses have years of experience in the hospitals so they can take very good care of the residents. The aides are caring and will go the extra mile. The food is great, if the kitchen can honor a request they will. The social worker helped with the discharge and provided a ear when I needed. I enjoyed the crafts and activities. They helped pass time when

I am certain that when you are making the choice on rehabilitation facilities or long term care facilities for your loved ones or yourself it is important to you that you choose the best that is out there. Especially during these times of crisis during a pandemic it is crucial that the staff caring for you or your family is top of the line. I can not speak for the past. I can only speak of now and the future. We have top of the line nurses who are highly skilled and educated. I myself have 26 years experience in the Emergency Room. I am mastered prepared and nationally certified in Emergency Nursing

I recently began working at The Diamond and I can already see that the team cares about their residents. They take the time to develop positive relationships with each resident and make them as comfortable as possible during their rehabilitation or transition to long term care. The therapy department is great and the new rooms look amazing!
