Services
Nassau Rehabilitation and Nursing Center is a state-licensed, Medicare-certified skilled nursing facility that offers a variety of services, including physical, occupational, and speech therapy, wound care, and medical/nursing. The clinical needs of an individual are evaluated by a multi-disciplinary Admission team to assure that each patient/resident receives the specific care identified by his/her physician.

Nassau Rehabilitation and Nursing Center's Adult Day Health Care is an alternative to nursing home placement where the frail elderly and/or disabled young adults who are being cared for at home to spend time away from home in a supportive environment.
At our program you will have opportunities to socialize with others in our attractive day room or outdoors on our patio, engage in meaningful recreational activities and hobbies, go on community outings, and become involved in community projects.Our Adult Day Program has been in operation for over 11 years and has a wide range of services that are offered.

Our dietary department, under the direction of our licensed Dietitian and Food Service Manager create menus and oversee the preparation of delicious meals. Chef-inspired meals exceed all daily requirements and we offer a selective menu with 3 choices every day. Our Registered Dietitians are involved in every step of the meal-planning process to ensure that the daily nutritional needs of the residents are met.
We specialize in programs that target the needs of residents with varying degrees of ability, memory loss or cognitive impairment. We recognize each resident's individuality and strive to maximize their potential for social fulfillment by learning about their life and past interests. Our Activities department provides residents with customized support and social stimulation targeting their specific needs regardless of their capabilities.

Miss Antoinette is the sweetest always takes great care of my dad. The whole facility is great. All the staff and even though at times they are short staffed they try their hardest and do the best job. Shout out to nurse Nicole she is the best always sweet and patient and takes great care of my dad.❤

This place it's horrible. My loved one died because of these people. Please don't put your loved one here. Please listen to the people that have written negative reviews. There are no good nursing homes in Long Island or anywhere. There all horrible. I wish we had never put our loved one in this place.

The facility is very clean and the staff is very caring. They try their best to make you comfortable and meet your needs. Joe from therapy has given me all the therapy I need and is also a friend. Sandy is very good at taking care of people. Yoni is very caring and and has taken care of all my needs. He has been extremely helpful and polite and if anyone needs any help you ask him and he will see if he can help you.

Words cant describe how grateful I am for the staff at Nassau Rehab. After being in the hospital for a month after having a stroke the hospital told my family to pick a rehab facility for me to go to. They were overwhelmed with choices especially because I was not vaccinated and they were concerned with me getting COVID. They decided on Nassau Rehab because when they toured the facility with Alaina from admissions they saw how clean the place was and she informed them there was no COVID positive patients and my family could come visit me. I wouldn't put anyone that I love in long term care. I just spoke to the health department commissioner's office and they told me that visitations should be fully back the way they were before Covid, but it's up 2 the discretion of the owners and administrators. Imagine that! If these people aren't letting you back in and letting you take your loved one out when you want to, ASK YOURSELVES WHY? All restrictions have been lifted in the state of NY. There is no reason why you shouldn't be allowed to go back to unrestricted visitations, go up to their rooms, spend as much time as you want with them

I do not recommend this place for long term care. My mother was there for a couple of years. They neglected to feed her for 2 1/2 days when she first started using a feeding tube. Many of the aids I encountered were nice but there were a couple who would leave her in dirty diapers. Her two main aids were attentive, kind and good. The facility didn’t provide any activities on her floor. The only thing they did for activities was paint her nails. Residents sat in wheelchairs all day long in the same room they ate in. Most of the nurses were good but there was one in particular who should have never

My name is Ricardo Rodriguez I was a patient at Nassau rehab. I was skeptical on going into a rehab during this pandemic but I felt safe when I arrived at the facility. The facility was very clean and the staff were friendly. Although I had many struggles with little motivation the Rehab staff were amazing on helping me reach my goals. Cheryl helped me get Medicaid and addressed all my concerns during the process. I’m home and doing well thanks to Nassau Rehab.

My mother Ana Martinez was admitted for short -Term. The recovery process was hard but with the support of the rehab team my mom did a quick recovery. she was able to make back home to us.. Special thank to all Rehab staff and CNA’s who took Care of my mom. I would recommend this facility to anyone who needs rehab
