Performance Physical Therapy & Sports Lab
Performance Physical Therapy offers traditional postsurgical and injury rehabilitation using a combination of manual techniques, functional movement analysis and therapeutic exercise. The Sports Lab component of our business provides personally tailored and comprehensive training programs to focus on sport specific biomechanics, injury prevention, and sports performance enhancement from a physical therapy angle.

Performance Physical Therapy works with many of our local youth, high school, collegiate and professional athletes to provide stretching, warm-up/cool-down protocols, athletic taping and assessments for injury prevention both before and after games. Performance Physical Therapy staff have a Titleist Performance Institute (TPI) Golf Level 2 certification which can help individuals improve their golf game.

Performance Physical Therapy and Sports Lab opened its doors in early 2013 to fulfill Calvin's dream of starting a physical therapy practice that delivers care to orthopedic patients and athletes in a more honest and ethical way. With the changing healthcare environment, clinicians in all areas of medicine are burdened with adapting to a restructured reimbursement plan.
Calvin earned his Bachelor of Science degree in kinesiology with an emphasis in rehabilitation from Concordia University in 2004 and went on to obtain his certification in strength and conditioning in 2005. After several years of working as a Physical Therapy Aide, he returned to California State University at Long Beach to complete a Master of Physical Therapy degree in 2010.
Performance Physical Therapy offers traditional postsurgical and injury rehabilitation using a combination of manual techniques, functional movement analysis and therapeutic exercise. Patients can either be referred by a physician or can self-refer without a doctor's prescription. There will be an initial evaluation where a history and physical examination is performed to establish a treatment plan to be carried out in subsequent visits.
An individual's insurance may cover up to 12 visits before requiring a prescription. Any subsequent visits that may be needed to treat the injury will need a prescription. If you feel that you have an injury or health concern that may benefit from the skilled services of a physical therapist, please contact us to see if your insurance is in compliance with this new policy called direct access.
Performance Physical Therapy accepts most major insurance carriers and participates with many local provider networks. Your physical therapy benefits will be verified prior to the start of treatment so that you are prepared for any financial obligation that you may have. We are happy to provide competitive cash rates and payment plans in the event that your insurance benefits are minimal or exhausted.

I can’t speak highly enough about Performance PT. Calvin, Katie, and their aids are so accommodating and helpful! The rehab process has been a breeze under their treatment. Their preventative treatments have saved my neck and the exercises I do daily under their direction have made a higher impact on my day to day comfort. Highly recommend any and every person looking for a fantastic PT clinic to come here! They’re well versed in all aspects of sports for athletes too!

Just be careful of cancellation fees on the same day. They charged more than other physical therapists. So do your research before picking them.

Calvin and his team are hands down the best in the business. They have helped my family recover from injuries and created general physical fitness routines that have proven very effective. Very professional office and PT organization. I highly recommend them!!

Katie and Calvin saved my back and my golf game! I found Calvin through the Titleist Performance Institute. I had started playing golf again after a multi year break and was loving it, but after a round I had severe back pain for days following a round. Katie provided a personalized assessment PT plan and got me back on track physically while Calvin's TPI assessment identified the body mechanics that were the root of my pain. Also, i can't forget their support/office staff, as all of them are awesome to work with!!!!

How to write a review that explains how good you guys are... I have written about 8 reviews and deleted all 8 of them because I have not felt they said enough. You guy truly understand the body and how to get it back in working condition! You meet the client where they are physically and emotionally with encouragement, patience, respect and care. In my opinion you cannot walk into Performance Physical Therapy with out feeling like you improved physically and made a friend. For the clients that want to truly improve, Performance Physical Therapy will challenge you and push you but give you the support
