A team of rehabilitation professionals, in conjunction with the physician specialist (orthopedic surgeon, geriatrician, neurologist, cardiologist, pulmonary specialist and / or hematologist), work with the patient and family to plan and implement an individualized plan of rehabilitative care. Physicians: All physicians are board-certified in medicine.
Recovering from a recent hospital stay following an injury or illness can come with many challenges. Often times, a family member, based on their experiences caring for loved ones at home, believe that certain changes require hospitalization. However, while true for the elderly living at home, one of the advantages of having your loved one in our care is our ability to respond and manage virtually any need they have - now and in the future.
I am so glad my father is a resident here. Words can't express how grateful I am to the entire staff for ensuring that he stays engaged, healthy and happy. Thanks for all that you do! Mom is so happy and thankful that we moved her here, and we're very thankful to the staff and the residents for being so understanding and helpful.

I lived here for my accident- bone fractures.
I was so grateful for Ari help me. After my hand surgery for over 30 days, I could not visit any Drs. I didn’t know who I can ask in this center. I went to Ari Administrator, He found out: It’s my insurance came too late which delayed me to see Drs. He then brought his friend Dr. , did postoperative management for me. I was so glad for the treatment.
At beginning, when I came to the center, I was so frustrated by ignorance from some housekeepers, who can’t answer any my concerns.
Finally I realized I have to go to the right person who can help

This nursing home is unprofessional and dirty. They lost my mother clothes and didn’t give her the proper care that she needed at her stay. She lost plenty of weight and constantly gave her Benadryl for her “itch” . I ask the nurse and cna questions and they couldn’t answer my questions. The front desk was the only one that had proper etiquette. I wouldn’t recommend this facility to anyone !

Clearly these 5 star ratings are fabricated. Do not send your loved one to this facility. My 98 year old grandmother is currently there and she is being treated poorly. The clothes we brought her have mysteriously disappeared and she constantly complains about being cold. The aides are very nasty and aggressive with her. And they never seem to have enough people on staff so they do not answer call buttons. She fell out of her wheelchair and it took almost 20 minutes before someone came to help her. My heart is hurt over this because seniors deserve to be treated better than this!

Along with many previous times, I was very impressed with my Mom’s appearance & overall well being when I visited her 7/8/21. I would like to thank Ari the Administrator for doing an excellent job of overseeing the Facility. I would also like to thank my Mom’s direct caregivers for extending such loving care. Do not send anyone you love here. I wish I could turn back time, but too late now. Horrible care. My mom asked for a bed pan and was told, "Oh no, honey, you have a diaper, and that's where you go". She then asked to be cleaned up, and was told "you don't always get what you want". Brought my mom food, and it disappeared weekly. Worst place ever!

The worst place ever ...nurses don’t answer the phone, a nursing supervisor lied that my
Mother was ok and she was not . My mother was there less then 72 hours both times and if I didn’t insist and argue with them to call an ambulance they would of let her suffer in pain.
Sadly because of Covaid you aren’t allowed to see them and need the staff to make you feel comfortable and let you know how your parents are and they did the opposite.

My mother came here for short term rehab after a bad fall. She has a variety of chronic conditions and needed subacute rehab and wound care. I cannot say enough good things about this place. They are MIRACLE WORKERS. We were in another nursing home in the past and it does not compare to this place. The staff are very warm and encouraging. My mother can be very stubborn but they got her to engage. We thought we may need to place her long term but after about 2 months SHE WAS WALKING INDEPENDENTLTY! I would highly recommend this place to anyone!!

My mother was recently admitted there for rehab. The staff was courteous and showed my mother respect during her stay for 3 months. I would like to thank the physical therapy , nursing, social worker and housing keeping staff for the wonderful service you all have provided. I would also like to thank Gail and the administrator Mr. Nate for making sure my mother was ok and comfortable during her stay at the nursing and rehab facility.
