At Willow Park Rehabilitation and Care Center we have gone well out of our way to assemble a medical team capable of delivering the comprehensive skilled nursing and rehabilitation services you need to recover your quality of life. Our therapist provide personalized and flexible treatment plans, one on one treatment, and patient-focused goals.
Services
The Willow Park Occupational Therapy Department provides therapy to a variety of patients with a focus on independence, self reliance and achieving maximum potential. The goal of the occupational therapy department is maximum function in the least restrictive environment.

The Speech / Language Pathology Department at Willow Park provides a variety of patient specific treatment programs for patients with speech / language, cognitive linguistic and swallowing disorders.Diagnosis begins with a thorough swallowing evaluation to identify the type and severity of the problem.

I loved it here (at first) but staff is under paid and unappreciated. Workers that put in 110 plus hours are being fired while employees who won’t even help the nurses are getting promoted and praised. This place can not keep staff and it because of the way they treat employees. The best employees are no longer working here.

Please don't put your loved one here. They don't answer call lights for the residents, they have to wait over an hour! Bed sores, why yes of course when you leave resident sitting in their own urine and feces for hours a day, day in and day out. You can never speak to anyone in charge... a nurse, a case manager, a social worker, Director of nursing....and if you actually catch some one they do NOTHING! "Oh let me check and we will get back to you" and then never to hear from them again. When we could get in the building before Covid, we would watch them ignore the phone, or

Absolutely horrible place....staff does not take care of patients....in the patient bath room the soap dispenser is filthy....looks like it has not been cleaned in several months...I would not recommend this place to anyone. We finally moved my family member to another facility....this place should be turned into the health department.....terrible....terrible place...I would give it a negative 1 star.....

Nothing but Compassion, and Respectful care for my mother here. The staff is upbeat, and friendly to residents and visitors alike. The facility is clean and welcoming.

Terrible care, the Director of Nursing, Megan is one of the rudest, selfish, non caring nurses I have ever met. She never returns phone calls, she never answer phone calls, my mother sat in her own urine for over 24 hours, I had to move my mother once again. The food is horrible, they should be ashamed. I will be reporting this to the state. Please do not place your loved ones here.

Wonderful and caring aides and nurses. The place is clean and everyone is friendly. Everyone is willing to help and it is a very nice facility.

Excellent facility. I had my grandmother here and she had a rapid recovery. Sweet aids that are willing to always help. Food is great, and I am super picky. Would recommend to all of my family and friends.

My mother came to Williow Park Rehabilitation Center after being in the hospital. The facility is very clean and modern. The food the majority of the time was excellent and allow personal selections. We were very pleased with physical therapy, occupational therapy, and speech therapy. This is a facility that is very understaffed. The staff members are by the book and always follow the laws. They are not compassionate or caring. They do their job and only the basics. There was no kind relationship building. They did not welcome us in nor say goodbye when we left. There was no dismissal plan other

Do not place a loved one in this place. My mother in law was placed here and within a month we moved her. Wasn’t getting showers, was served food that was cold. Was left in Fort Worth at an appointment and we had to go get her. Noticed her feet swelling and realize she wasn’t getting her meds. Questioned staff and they found they were not giving meds and falsifying their documents. Steer clear as we have contacted the state
