If you wish to bill out-of-network with another insurance company, payment in full is expected upfront; you will be provided a superbill to submit to your insurance carrier for reimbursement. Medicare requires a physician referral prior to your appointment; referrals can be faxed to: 1 (833) 703-0207. If you have a different insurance company and would like to submit for out-of-network reimbursement, you will be provided with the necessary paperwork.
