Services
We provide Patient Centered Medical Home model of care. The PCMH is a health care setting that facilitates partnerships between individual patients, and their personal physician, and when approprite, the patient's family. The medical home is responsible for all the patient's health care needs or taking responsibility for appropriately arranging care with other qualified professionals.
Our physician, Dr. Farhath Dr. Kareem is committed to providing comprehensive and compassionate care to her patients. She addresses both prevention as well as treatment of illnesses, physicals and often include in office labs. Dr. Kareem completed residency in 2004 from Moses Cone Hospital/University of North Carolina and KCMS /Michigan State University.

Dr. Kareem is an excellent doctor for my family. Very patient and kind, plus the biggest benefit is that she actually listens to the patient. She knows the history and is not a drug pusher; always concerned. And she actually calls the patient when sharing test/lab results, something that has become very rare. If you are looking for a doctor 'the way they used to be' (caring, take their time, listen, follow up, etc.), this practice is for you.

I am very thankful to have chosen Dr. Kareem as my primary doctor. She is very caring and compassionate about helping you to get better. When I was in the hospital in the emergency room she call to check on me. Also, she contacted me when I got out and follow up with an appointment week later. So I am pleased to have doctor that is to the point and cut no corners when it comes to your health. Office staff is wonderful too.

I have been going to Dr. Kareem since 2012. I was nervous about going to a new doctor because I had such a good repore with my previous doctor for so many years in NY.
Dr Kareem helped all my fears go away she was just as good as my previous physician.
She is on top of everything whenever I have a problem she is on it at finding the solution.
I recommend Dr Kareem to everybody I know.
She has excellent bedside manner an amazing memory and is very understanding and thorough.

I have been in Raleigh for 10 years and in that time I have switched doctors a many of times because of the lack of care and test I should have had done!
Dr kareem and her staff are amazing! She is very through and I have finally found a doctor I will be sticking to! Cannot say enough good things about dr kareem !!!

There are not many doctors in the Triangle that will work you in the same day--which I believe displays Dr. Kareem's commitment to her patients. As a result of those that she has "worked in" to be seen the same day, I have seen her keep her office open well past her closing hour to ensure that everyone has been seen to include Saturday. The staff are passionate about customer service and all very accommodating which well represent Dr. K.
