It is being patient centered/oriented and the patient is included as a member of the treatment team. Our name is Louisville Patient Centered Medical Home, PSC. We are now the first private family practice doctor's office to get the highest level 3 (tier 3) in Louisville, KY to be recognized by the NCQA (National Committee for Quality Assurance) located in Washington, D.C. Services
Telehealth is defined as the remote diagnosis and treatment of patients using telecommunications technology. It means conducting visits using audio and video over the internet, rather than meeting in the doctor's office. Call (502) 896-8041 to enroll now. The doctor/provider will open the application on their end and we will be able to talk to and see each other.

Edit: it has been a year and I really can’t stand this place anymore! I’m switching doctors ASAP. The ladies on the phones are rude and disinterested. It’s all I can do to get them to answer my questions. That combined with the aggressive pushing of Pap smears when my gynecologist is handling them. Also their doubt of my diagnosed mental disorder a couple years ago and their reluctance to give me medicine that now two years later has vastly improved my life. Do not go here!! You get what you pay for and you’re not getting anything good here!

From the time I walked in the office I did not feel comfortable. The first 2 visits were 10 minutes at most when I had a list of concerns, andIi was rushed out without any concern for my health. All they want to be is a hub to send you out to other doctors. And they can't even do that any skill. I have had to re-schedule a procedure twice because the referral department did not do their job correctly. And when I tried to talk to them about it they were very rude and talked over me. Look fr another office before you go here.
