Our team works with your doctor to provide the care that you need and deserve in your home or residential care facility. In coordination with your doctor, we create a home health care plan specifically designed with your needs in mind.

Services
Nursing & Rehab At home is a Medicare-certified, State licensed agency and a member of the California Association for Health Services at Home (CAHSAH) and the National Association of Home Care and Hospice (NAHC). Nursing & Rehab At Home was formed following the closure of Stanford Home Health where many of our leadership and clinical staff were previously employed.
Doctors and hospitals choose us for our individualized patient care services and optimal outcomes. Assistance in community resources education for financial, social, and/or transportation issues. Registered Dietitian assists with nutritional supplementation and meal planning in coordination with specific instructions by your physician for disease management and/ or suggestions by the Speech Therapist.
Medicare pays 100% for patients who meet the eligibility requirements for the Home Health benefit. Should you not meet these requirements, we may still be able to provide services at the patient's home under the Medicare Part B benefit or through private pay arrangements. Many of our services are also covered through other health insurance providers or workers' compensation.
Nursing & Rehab At Home is always looking for qualified clinicians and support staff to join our growing team! We currently service San Mateo County, Santa Clara County, San Francisco, and Alameda County with Full Time, Part Time & Per Diem positions available. My 86 year old father broke his hip and Nursing & Rehab at Home provided his post operative care. They were excellent! The RN’s, Swati and Gabe, were wonderful to dad. OT Beth and PT Lori were caring and very positive through out my dad’s recovery. Dad met all of his goals with their encouragement and hard work. I highly recommend Nursing & Rehab at Home for your home health care needs. We will miss seeing our “team. “
