The Rehabilitation Care Program at Forest City Rehab is a unique comprehensive program designed for individuals whose health has been temporarily interrupted due to an illness, injury or recent surgery. Our program is designed to maximize the functional abilities of our residents, enabling them to reach their highest level of independence and to accommodate individual goals and capabilities based on tolerance levels.

It’s easy for terminated staff to leave bad reviews about Forest City but when it comes down to it if you did your job you would still have one. Forest City is not only a nursing home but it’s centered around behavioral health- that alone makes everyone’s job a little harder. I absolutely love my job and everyone I work with. If you truly enjoy working with the elderly and anyone with mental illness and want to make their day better then you look past that our building may be “older” and not as “up to date” as the building down the street but the staff that we have looks at our residents

I am sharing only a one star just because they accepted and had my mother in law there as a patient. They are poor staff. They never returned calls or made sure my mother in law phone was charged for us to call. We call and they would say they will charge it a whole month went by and nothing. They never updated on how her P.T. was going. I give props to Kendra one of the staffs as she was helpful and to Amanda she was very rude. I do not recommend this place to any one at all. When it was time for discharge which was recently they have lost her items. She did not come back with a phone/charger

This facility is lovely. The people who come to take care of me are lovely and friendly. I have a comfortable bed and get to work directly with Physical therapy workers. I have social workers who visit with me often. The facility is doing all they can during a hard time to make sure I don't feel lonely.

Forest City deserves a 5-star rating in my book. The staff is friendly and helpful. If I have a problem I know I can to social services and they will be there to help me out or find a solution. Social Services, Housekeeping, Nurses, C.N.A.s and the administration here is fantastic. Everyone puts forth 100% to make this place shine.

Forest City Rehab and nursing can accommodate long, short term, and respite care. Employees from the frontline to the executive management to the owners want the best for our residents. Employees are constantly looking for new ways to provide residents with individual patient centered care and activities. Even with COVID restrictions in place residents experience a peace of mind and well being on a daily basis.

My sister was a patient at Forest City Rehab, from the end of December 2020 to February 18, 2021.
From the stories my sister tells me to what I experienced trying to get help for her during her stay there, wasn’t very good. I was told half truths concerning questions that I would ask them concerning my sister. I feel like some of the staff do not care about the residents there. They do not have time for them. Angie, the Social Worker, has an attitude problem, the way she talk to my sister and I. I see in the other reviews how the reviewer say how nice and helpful she was to them. She wasn’t

Like any Nursing home they have there ups and downs. My brother was a resident there for 8 years and considered it is home. He made many friends there and had his card group and even had a nice lady friend there. My brother was not the easiest person but they handled him very well. Tony Valentino the administrator was wonderful to deal with and they had a very good nursing staff.

I was a patient there for six weeks from the end of February 2019---April 2019. I was there for mental health rehab and stabilization. The staff there are extremely nice and competent. Everyone was friendly. The food was good and there was a wide variety.
Housekeeping staff came and cleaned my room daily and were accommodating to my needs, also respecting my privacy. Stacy Abbot worked with me and on my behalf to file for Medicaid. She was extremely helpful and courteous. Always glad to see me and never made me feel like I was a burden or interrupting her, even though I knew she was busy. Angie
