Klaire Adee, PT, DPT is a graduate of Northern Illinois University, where she earned her Doctorate of Physical Therapy degree.

Belvidere Rehab & Sports Medicine was founded with the mission of providing top-level skilled physical therapy in a relaxed environment. Over the years, BRS has grown, adding treatment offerings and specialized therapy equipment. Why? Because we are constantly changing and growing to better meet the needs of our patients and referring physicians.
Our certified physical therapist covers a wide range of services. How does it all work together? It all begins with a comprehensive evaluation. Every new patient is evaluated extensively to determine the best mode of care, including a customized care plan that is developed for each patient. Each patient and each injury are unique, and so is our treatment plan for that patient.
A Physical Therapist specializes in therapy programs for musculoskeletal injuries and disorders, sports injuries and post-operative rehabilitation. If your physician refers you for therapy, you have the right to attend therapy wherever you wish. Certain therapists are more qualified to treat certain conditions.
Form 2, Privacy Notice, is for your information only. Forms 3-5 can be printed, completed, and brought with you to your first appointment. A list of all over the counter as well as prescribed medications including dosages and the frequency that you take each medication. If your injuries are as a result of an auto accident, please bring with you a copy of your Accident/Police report.
We will bill your insurance given we have been provided the information on a timely basis. If we are contracted with your insurance, you will be sent a statement after we receive an Explanation of Payment/Benefits. If we are not contracted, understand that only you have a relationship with that payer.
